For families who wish to request a mask exemption for the second semester of the school year, the form will be open on Tuesday, January 18 at 7:00 a.m., and will close on Tuesday, February 8 at 5:00 p.m. Families will be notified of acceptance by the end of day on Friday, February 11, and exemptions will be effective the following Monday (February 14).


This form is only for new applications. Those who already have an exemption in place do not need to fill out the form again.

As a reminder, mask exemptions apply to students only during the regular school day, and do not supersede mask requirements of any public health or other legal authority having jurisdiction (ie: school transportation per federal law).

In addition, this exemption does not exempt students from the contact tracing guidelines as defined in the Ohio Department of Education's Mask to Stay, Test to Play policy. The district will continue to follow contact tracing and other safety protocols at the direction of public health experts at the Ohio Department of Health and CDC.
